All personnel are screened, trained, certified, and bonded. CCCSO conducts reference checks and criminal background checks. Employees must pass a drug screening test. SENIOR SOCIAL SERVICES: > Assessment: To determine clients' needs and provide linkage to services. > Health Preventive Services & Screening: Free blood pressure, vision and hearing tests. > Reassurance Monitoring: Daily phone reassurance. Also, see information about personal emergency response system below. > Project Lifesaver (304-529-0393): Registration intake at CCCSO for electronic personal locator system in a non-removable bracelet on the wrist of any person with Alzheimer's and other Related Mental Dysfunction Disorders. Funding through the Pilot Club and search/rescue conducted by the local, designated emergency response teams. Bracelets are applied by the emergency response team for those meeting eligibility requirements. Cost: $250 plus monthly $12 fee for private pay. Charitable assistance available for income eligible Cabell Co. residents. > Application Assistance (304-390-0020): Assistance with Medicare paperwork > Medicare counseling (304-390-0020): Assistance with application process for Medicare cards and Medicare Part D. > Provides vouchers for the Farmers Market, as available. > Recreation: Operates senior centers, providing nutrition services, health screening, exercise programs, cards and games, trips and tours, information programs. > In-home care providers (see below) SENIOR CITIZEN CENTERS: > CECIL H. Recreation (exercise, musical activities, recreational program, Bible study, and ballroom dancing) Blood pressure clinic. Free computers and internet access. Persons 60+. Services: Meals by donation, recreation (crafts, exercise, field trips). Free computers and internet access. Coordinator: Linda Porter FOOD: MEALS ON WHEELS (304-529-4952): Prepared at CCCSO, 724 10th Ave. Eligibility: Persons 60+ years who are home-bound, at risk and/or frail. Requires a physician's referral of need. Cost: By donation. Services: Provides one hot, mid-day meal/day for 5 days. (7 when necessary) LIQUID NUTRITIONAL SUPPLEMENT (304-529-4952): Provides nutritional supplement Ensure at a reduced rate below normal market price. HOUSING: Eden Park, Inc. (304-529-4952): 732 Tenth Ave. (20 units for persons 62+ yrs, meeting eligibility guidelines) HUD subsidized. TRANSPORTATION: > For ambulatory clients 60+ years: Transportation to senior centers, nutrition sites, grocery stores, etc. Sliding scale fee and by donation. Must request ride one week ahead of need. Escort service for obtaining medicine, groceries and to doctor appointments. > For persons needing "hands-on" transportation assistance: provides a handicap accessible vehicle on a sliding fee scale. LIGHTHOUSE PROGRAM, CABELL COUNTY: Eligibility: Persons 60+ yrs.who are medically eligible based on an evaluation by the CCCSO R.N. Must be unable to perform two "levels" of activities on their own and meet income guidelines. Maintains a waiting list. Services: Provides 60 hours/month of in-home services such as light housekeeping, personal care, mobility assistance, meal preparation, etc. SENIOR SUPPORT SERVICES: 60+. Provides light housekeeping services on a weekly or bimonthly basis. To help keep seniors in their own homes. HUGE WAITING LIST Requested contribution for services. MEDICAID WAIVER PROGRAM (304-529-4952): Eligibility: Disabled persons 18+ and seniors in Putnam, Cabell, Wayne, Mason, Lincoln. Must have referral from a Medicaid Waiver Casemanager casemanagement agency and had an assessment & eligibility determination for PAS-2000 nursing home. Cost: No cost to clients currently receiving Medicaid. If not on Medicaid , a spend-down must be met. Services: Personal care, shopping, laundry, and limited housekeeping services. MY FRIEND'S HOUSE ADULT DAYCARE (304-529-0393): 632 Ninth Ave., Huntington, WV 25701. (Located at the Cecil H. Underwood Senior Center) > Cost: Private pay, private insurance and scholarships. > Eligibility: Requires a physician's order showing physical or mental diagnosis requiring supervision. Must be ambulatory. Pre-admittance screening required. Reservations recommended. > Services: Activities and field trips. Many activities geared to persons with dementia and Alzheimer's. Personal care services available on site for additional charge. FAMILY ALZHEIMER IN-HOME RESPITE CARE (F.A.I.R.): Provides 16 hours of care/week for persons with a physicians Rx of dementia, Alzheimer's or related diseases. SUPPORT: Alzheimer's Support Group (304-529-0393) MONITORING SERVICES (304-529-4952): > Personal emergency response system (medic alert) $50 installation fee plus $30/month.
