Eligibility: Homeless individuals, persons at risk of becoming homeless and those organizations serving the homeless. No geographical limitations.
Description:
COALITION ORGANIZATIONAL SERVICES:
Develop long/short-term housing options. Coordinate & enhance coalitions and services between and among social service/health organizations interested in serving zero, low-moderate income individuals/families at risk of homelessness.

HARMONY HOUSE (SERVICES FOR HOMELESS INDIVIDUALS):

> Shelter: Screens and refers for emergency shelter through the Huntington City Mission, Branches, and the WV Dept. of Health and Human Resources.
Also assists with long-term shelter solutions.

> Dental Care: Free dental clinic operated by Valley Health Systems. Serves the homeless and under some circumstances will serve persons in subsidized housing whose families are zero income or doubling up for less than six months.

> Valley Health - Homeless Health Care: Free health clinic for persons who are homeless. Under some circumstances may be able to serve persons living in subsidized housing whose families are zero income or doubling up for less than six months.

> Mental Health: Counseling and outreach for homeless individuals provided by Prestera Center.

> Case Management: Provided by Information and Referral and the WV Dept. of Health and Human Resources.

> Payee services

> Education: Computer training

> Additional Services: Clothing, transportation, household/ personal needs and outreach. Short-term storage lockers for persons living outside. Serves as mailing address for homeless individuals. Payee services. Showers, washer/dryer with detergent. Access to phone for local business-related calls.

COALITION HOUSING PROJECTS:

> Fifth Avenue Apartments (525-8417): 5th Ave. and 9th St., Huntington, WV 25701. 53 apartments for low-income and disabled individuals/families.

> Project Hope (523-0293, ext. 308): 1012 Seventh Ave., Huntington, WV 25701. Provides pass- through funding and funding oversight for social services coordination of transitional housing for families. Referrals for this housing can come from the Huntington City Mission, Branches or other social service agencies.

> Vanity Fair (523-9487): 621 4th Ave., Huntington, WV 25701. Subsidized, single-room-occupancy housing for single people.

> Seaton Taylor Apartments (523-9487): 402 Seventh Ave., Huntington, WV 25701. Subsidized apartments for the mentally handicapped. Tenants receiving supporting services via agencies that place them in this housing.

> Heistad House (781-2261): 534 Seventh Ave., Huntington WV 25701. Permanent housing for the chronically homeless. Required to cooperate with the Housing First Team.
