| Description: | THE AGED AND DISABLED MEDICAID WAIVER PROGRAM: Provides case management for in-home assistance with activities of daily living. (instead of in a long-term care facility) Guidelines differ from regular Medicaid eligibility requirements. Provides case management, assessment, care planning services, monitoring, reassessment, homemaker services. Arranges referrals for medical equipment and supplies. PERSONAL CARE PROGRAM: Case management for personal care needs of persons on Medicaid and meeting a medical need criteria (such as recuperation from surgery). WV SELECT: Private pay casemanagement services for in-home care. Individualized planning. |
