| Eligibility: | Cabell, Wayne, Lincoln, Mason, Mingo, Putnam Co.WV; Lawrence Co. OH; Boyd, Carter, Greenup Co., KY. Persons below 150% Federal Poverty Level, with no medical insurance, ineligible for Medicare/Medicaid. See narrative for other exceptions. |
| Description: | ADDITIONAL ELIGIBILITY EXCEPTIONS: > Must live at a permanent address. > Can serve persons with only Medicare Part A, in the clinic but not for prescriptions. > Can serve persons with private insurance that covers only hospitalization in the clinic, but not for prescriptions. > To be eligible for the Women's Breast & Cervical Cancer Screening Program, patients must fall below 200% of the Federal Poverty Guidelines and have no insurance. > Can serve non-U.S. citizens if they are in the U.S. legally and meet all eligibility guidelines. Would need letter from DHHR indicating they are not eligible for Medicaid. PROCEDURE FOR APPLICATION: Call or visit the clinic during office hours to pick up application packet and list of required documentation. Once 100% completed, you must hand deliver it to the clinic on the first, second or third Monday of each month between 8:30-10:00 am. CLINIC SERVICES: > General medical care > Specialty Care: Cardiology, Neurology, Gastroenterology, Endocrinology, Podiatry, Nephrology, Counseling, Orthopedic Care, Chiropractic Care, Dentistry, ENT and minor surgery. > Female cancer screening provided by nurse practitioners or physicians. Women's Breast/Cervical Cancer Screening: M 1 pm-4:30 pm > Referrals for established patients to specialists serving the clinic (or others when needed). > Referrals for established patients for free counseling/psychological counseling at the clinic. COMMUNITY PRESCRIPTION ASSISTANCE PROGRAM: Hours: M-Th 8:00 am-4:30 pm Eligibility: Ebenezer patients, indigent patients of St. Mary & Cabell Huntington Hospital and private patients of volunteer providers. The private patients must be at or below 150% of the FPL. Cost: All patients are requested to make a donation of $3/prescription. If a 3 month supply is received, a $9 donation is requested. Persons unable to make a donation are still provided medication. DENTAL SERVICES: Provides limited dental care for already established patients of the clinic. Long waiting list. EMO CARD: Provides labwork, x-rays, tests through St. Mary's and CHH, free of charge. For persons at or below100% of the FPL. HEALTHY LIFESTYLE - HEALTHY LIFE (304-529-0753, ext 118): Eligibility: Patients with chronic diseases, primarily diabetes. Must have a referral from a physician or nurse practitioner. Services: Seven week education class, cooking schools, support groups, and exercise program. Opportunity to earn "Healthy Bucks" to be used at the Fresh Food Project or Market. FRESH FOOD PROJECT (304-529-0753, ext. 142) : To provide gardening opportunities to the patients by providing a prepared garden spot and a way to earn vegetable plants for their garden. FRESH MARKET, "Healthy Lifestyle, Healthy Life Market": Every Friday from 9 am-6 pm during the summer. Provides a a weekly opportunity in front of the Douglas Centre to purchase plants and vegetable at a reasonable cost. Available to patients and the community. HUNTINGTON'S KITCHEN (304-522-0887): Joint project with Cabell Huntington Hospital to provide instruction to anyone from the community interested in learning how to cook healthy, convenient and cost-effective meals from scratch. Originally created for the television series "Jamie Oliver's Food Revolution." OPERATION FRONTLINE (304-522-0887): Free healthy cooking classes taught at Huntington's Kitchen. Targets lower-income persons. Provides information about basic nutrition, a free cookbook, free meal of the dishes prepared in class and a free bag of groceries with the recipe ingredients after each class. Requires committment to attend all six sessions . SAVE OUR SISTERS: Outreach program utilizing volunteers to equip women in the community to promote good breast and cervical health. Functions also as a support group. CRUSAIDS: Community-based HIV/AIDS outreach that meets the last Tuesday at the Douglass Centre at 1448 10th Ave., 5 pm VOLUNTEERS NEEDED: > Save Our Sisters: Utilizes volunteers to serve as helpers, serve on committees and distribute information to recruit African-American women to get regular female cancer exams. > Clinic: Utilizes medical and clerical support volunteers. |
