ADOLESCENT HEALTH INITIATIVE -AHI (304-293-4692): Addresses the most prevalent health risks facing adolescents by empowering communities and supporting efforts that build resiliency and strengthen families.

AGRABILITY (800-841-8436): Assists farm families with developmental and other disabilities in obtaining services and technical assistance/assistive technology/devices.
Assists people of all ages with developmental and other disabilities to explore the technology they need to become and remain independent at home, work or school.

BENEFITS PLANNING, ASSISTANCE AND OUTREACH PROGRAM -BPAO (304-293-4692): Enhances community-based benefits planning and assistance services statewide for West Virginians with developmental or other disabilities. The goal is to increase the number of beneficiaries who return to work and achieve self-sufficiency.

COMMUNITY PASS (304-293-4692): Establishes and expands opportunities for personal assistance services that allow individuals to fully participate in the community and increase their knowledge and options for consumer control and direction of their personal assistance services and supports.

CRISIS SERVICES PROJECT (304-293-4692): Provides technical assistance and training for people who provide support to individuals who have developmental disabilities and who are at risk of or are having behavioral crises.

DISABILITY AND BUSINESS TECHNICAL ASSISTANCE CENTER -DBTAC (800-841-8436): Provides information, technical assistance and training to educational institutions about developing, buying and using information technology products that are accessible to staff and students with disabilities.

DISABILITY PROGRAM NAVIGATOR (304-293-4692) Navigators work within WORKFORCE WV One-Stop Centers statewide to make the resources available to individuals with disabilities.

FEEDING AND SWALLOWING CLINIC (304-293-4692): Offers an interdisciplinary team to assess the needs of infants, children and adolescents who may have feeding, eating and/or swallowing difficulties.

IMPACT 20/20 (304-293-4692): Prepares higher-education faculty to serve as leaders in infusing disability content and related issues into pre-service course work and practical experiences. Includes undergraduate and graduate Certificate Program in Disability.

JOHN CHAFEE FOSTER CARE INDEPENDENCE SERVICES ( 304-558-7980): Oversees independent living program for children exiting the foster care system at age 18.

MATERNAL AND CHILD HEALTH/LEADERSHIP EDUCATION IN THE AREA OF DISABILITIES -MCH/LEAD (304-293-4692): Recruits and selects health care professionals who are committed to serving children with special health care needs. The program's mission is to develop leaders who participate as team members and lead activities that promote family-centered care. Includes Core Course, Professional Development Seminar and Family Partnerships.

MEDICAID WORK INCENTIVE NETWORK - M-WIN (800-841-8436): Removes barriers to employment for persons with developmental and other disabilities through the enhancement and development of Medicaid services and related components that will enable people with disabilities to work competitively and sustain adequate health coverage.

NATIONAL SERVICE INCLUSION PROJECT- NSIP (304-293-4692): Provides a comprehensive program of training, technical assistance, and dissemination designed to increase the participation of individuals with disabilities in National Service.

NUTRITION SERVICES (304-293-4692) : Offers direct services to people of all ages and their families based on individual needs. Program staff can answer questions and assist with identifying resources for individuals with developmental and other disabilities, families and professionals.

PARENT NETWORK PROJECT - PNS (800-841-8436): Parents of children with special needs assist other families in getting the services, financial help, medical understanding, educational help and other supports the family needs. PNS staff conducts site visits, home visits and attend school meetings upon request. This project helps educate families and professionals about the valuable role that parents play.

POSITIVE BEHAVIOR SUPPORT PROGRAM (304-293-4692): Offers assistance to people who have challenging behaviors and developmental disabilities. Training and consultation are available to individuals, their families and support personnel to help prevent problem behaviors, to create effective community support and to encourage planning and life goals centered around the individual's needs. Provides referrals to potential funding sources (no direct financial services).

RURAL EDUCATION FOR APPALACHIAN COMMUNITY HEALTH AT HOME -REACH (877-724-8244): Provides interdisciplinary pre-service training for a person-centered approach to home assessments for individuals with disabilities, chronic conditions and the elderly.

RURAL HEALTH OUTREACH GRANT NEURO NETWORK FOR COORDINATION OF SERVICES -NNCS (877-724-8244): Assists people with neurological impairments such as spinal cord injury, multiple sclerosis and spina bifida in receiving a wide range of supports through service coordination that is effective, culturally sensitive and coordinated among service providers.

SPECIALIZED FAMILY CARE PROGRAM -SFC (304-558-0074): Recruits and trains host families to provide training, nurturing and family atmosphere for individuals with developmental and other disabilities who need full-time care. The specialized family care home is a place where individuals can grow and develop to their maximum potential mentally, physically, emotionally and socially.

SPECIALTY CARE COORDINATION (NURSING AND DENTISTRY PROJECTS) 304-293-4692: Provides specialty care support in the areas of dentistry and nursing.

SUPERVISORY STAFF TRAINING AND RESOURCE DEVELOPMENT PROJECT 304-293-4692: Provides technical assistance in the areas of staff development and training for child welfare and adult service workers.

THE FAMILY RESOURCE CENTER - FRC (800-841-8436): Offers disability related information resources, training, networking resources and emotional support to individuals and families.

TRANSITIONING TO INCLUSIVE COMMUNITIES - TIC (304-293-4692): Assists persons with developmental and other disabilities or those in need of long term care in making informed decisions about their living choices. (Real Choice/Nursing Home Transitioning)

TRAUMATIC BRAIN AND SPINAL CORD INJURY PROGRAMS - TBI/SCI (877-724-8244): Cabell, Wayne Co. residents, call: 304-720-3200 x 216) Provides services to promote functional independence and productive lifestyles for residents with traumatic brain or spinal cord injury. Provides screening for "Funds for YOU", which provides financial assistance as a last resort for brain injury patients to promote increased independence. Provides screening for TBI Clinic in Morgantown.

VISIONS (Visually Impaired Seniors In-Home Outreach and Networking) 800-642-3021:
Provides free in-home services to adults 55+ with severe vision impairments. Provides assessments for vision-related assistive technology, developes independent living plans and provides skills training in activities of daily living, home management, orientation and mobility/ communication. Provides low vision aids including magnifiers, large print items, etc.

WEST VIRGINIA ASSISTIVE TECHNOLOGY SYSTEM - WVATS (800-841-8436):
Assists people of all ages with developmental and other disabilities to explore the technology they need to become and remain independent at home, work or school.
Provides referrals to potential funding sources (no direct financial services). Has a free home assessment project for persons 62+ to determine how to assist persons to remain in their own home. May be able to assist with ways to obtain a wheelchair ramp.

WEST VIRGINIA: BUILDING INCLUSIVE ONE-STOP SYSTEMS -BIOSS (304-293-4692): Assures the integration of people with disabilities into the workforce investment system by building the capacity of comprehensive One-Stop Centers to provide programmatic access and a streamlined, seamless service delivery for people with disabilities.

WEST VIRGINIA-REAL CHOICE (866-720-3606): Creates enduring improvements in community long-term support systems so that individuals of any age who have a disability or long-term illness have the choice and necessary support to live and participate in their communities. WVATS/IMPACT 20/20 ASSISTIVE TECHNOLOGY FUNDAMENTALS TRAINING COURSE (800-841-8436): Increases the awareness and accessibility to assistive technology by implementing an assistive technology fundamentals training course for individuals with disabilities or chronic health conditions, family members of individuals with disabilities, and service care providers.

WV WORK INCENTIVES PLANNING AND ASSISTANCE PROJECT (304-293-4692, ext 1107): Provides information to Social Security Beneficiaries about how working will affect their benefits, food stamps and assisted housing.
