| Description: | SERVICES: > Case management, including Medicaid Waiver Program, for collaboration of client, family member, medical team and homemaker agency. > Homemaker agency assists in the home with personal care, preparing meals, grocery shopping, companionship, light house-keeping and transportation to doctors appointments. Also open to Medicaid Waiver clients. > Conducts state criminal background check on all employees. |
